Side-Lying Thoracic Rotations: Unlock Upper Back Mobility

If your upper back feels stiff from hours of sitting, driving, or working at a desk, you’re not alone. Limited thoracic spine mobility is one of the most common issues I see in people over 50—and it often shows up as shoulder tightness, poor posture, or even low back discomfort.

One of my favorite ways to restore movement and relieve that “locked-up” feeling is with a simple exercise: the Side-Lying Thoracic Rotation.


Why Thoracic Mobility Matters

Your thoracic spine (the mid-back area) plays a crucial role in rotation, posture, and overall movement efficiency. When it gets tight, the body compensates by asking other joints—like the lower back or shoulders—to pick up the slack. That’s when pain and dysfunction show up.

Improving thoracic mobility not only eases stiffness but also:

  • Enhances posture

  • Reduces unnecessary stress on your lower back and shoulders

  • Helps you breathe more freely

  • Improves performance in daily activities and exercise


How to Do Side-Lying Thoracic Rotations

  1. Set Up: Lie on your side with your knees bent at 90 degrees. Place your bottom hand under your head for support.

  2. Starting Position: Extend your top arm straight in front of you, palm facing down.

  3. The Movement: Slowly open your top arm across your body, reaching toward the floor behind you. Allow your chest and shoulders to rotate while keeping your knees stacked.

  4. Breathe: Inhale as you open up, exhale as you return.

  5. Reps: Perform 8–10 rotations on each side.

Pro Tip: Don’t force your shoulder to the floor—let your body gradually open up over time.
