Knee pain doesn’t just come from “bad knees.” More often than not, it comes from how the muscles around the knee are working together—or failing to. Two of the most important muscles in this equation are the vastus lateralis (VL), the strong outer quad muscle, and the vastus medialis oblique (VMO), the stabilizer on the inner thigh just above the knee.
When the VL dominates and the VMO isn’t doing its job, the kneecap doesn’t track properly. Over time, that can cause nagging pain, stiffness, and even long-term damage to the joint. The good news? There’s a simple test you can do at home to see if you’re at risk.
How to Perform the Seated Knee Tracking Test
Sit on the floor or on a firm surface with your legs extended in front of you.
Place your hand gently above your kneecap so you can feel its movement.
Press the back of your knee firmly into the floor—this contracts the quadriceps.
Watch the kneecap as it moves upward toward your hip.
What the Results Tell You
✅ Good tracking: If the kneecap moves in a straight line up toward your hip, your VL and VMO are in balance.
⚠️ Poor tracking: If the kneecap pulls outward instead of straight up, that usually means the VL is tight and/or the VMO is weak.
This imbalance may not cause pain immediately, but over time it can wear down the joint, especially during activities like walking, running, or climbing stairs.

The combination of release and activation restores balance and helps the kneecap glide properly again.
